How to apply taiwan visa for Seaman in the Philippines?

REQUIREMENTS FOR SEAMAN VISA
1. An online Visa Application Form (please access https://visawebapp.boca.gov.tw to fill up the Form and print them out)
2. Two (2) 1.5” X 2” head & shoulder photos with white background (taken w/in (3) months)
3. Passport valid for at least six months and old passport showing previous travels
4. Birth Certificate issued by NSO
5. Marriage Contract (if applicable) issued by NSO
6. Seaman’s book should be valid for at least six months
7. Seafarer’s Registration Certificate (SRC)
8. Letter of Guarantee addressed to Taipei Economic and Cultural Office (TECO) from Local Shipping Agency, indicating the following information:
a.) Complete name list of all seaman with their respective passport numbers and seaman’s book numbers
b.) Complete name, address and telephone number of the contact agent in Taiwan
c.) Complete name of the shipping vessel, date of departure and port of entry
※ NOTE: One guarantee letter per seaman
9. Letter of Guarantee from Taiwan Shipping Agent in Chinese letterhead to be sent thru fax to TECO and a copy to the local shipping agency, indicating the following information:
a) Complete name list of all seaman with their respective passport numbers
b) Complete name of the shipping vessel, date of departure and port of entry
c) Taiwan Shipping agent’s name in Chinese character, address and telephone number
※ NOTE: One guarantee letter per seaman
10. Overseas Employment Certificate and Contract of Employment
11. All documents submitted to this office should be Original and 1 photocopy
Visa Fee : P2,100 (NON-REFUNDABLE) payable upon filing of application Processing Time : 3 Working days
Expedite Fee : P1, 050 / Processing Time: 1 Working Day
Filing Time : Monday to Friday 8:45AM to 11:45 AM
Releasing Time : Monday to Friday 1: 45PM to 4:45PM

2. Two (2) 1.5” X 2” head & shoulder photos with white background (taken w/in (3) months)
3. Passport valid for at least six months and old passport showing previous travels
4. Birth Certificate issued by NSO
5. NBI Clearance
6. Marriage Contract (if applicable) issued by NSO
7. Seaman’s book should be valid for at least six months
8. Seafarer’s Registration Certificate (SRC)
9. Chinese approval letter of the Ministry of Transportation and Communications, Republic of China.
10. Overseas Employment Certificate and Contract of Employment
11. Medical Examination Clearance(Type П) from any of the following accredited Hospitals and Clinics:
a) Arguelles Medical Clinic, Inc.
b) Good Health Diagnostic Center, Inc.
c) Manila Doctors Hospital
d.) Our Lady of Fatima Clinic
e.) Philippine Medical Test System
f.) Physicians’ Diagnostic Center (Manila and Cebu)
g.) Ruben C. Bartolome M.D. Clinic, Inc.
h.) San Marcelino Medical Clinic
i.) S.M. Lazo Medical Clinic
j.) St. Martin Polyclinic, Inc.
k.) St. Peter Paul Medical Clinic
12. The applicant will be scheduled for an interview
13. All documents submitted to this office should be Original and 1 photocopy
