What are 2nd Engineer documentary requirements for Revalidation?

Reference: STCW Circular No. 2014-11, STCW Circular No. 2015-03, STCW Advisory No. 2015-21, STCW Advisory No. 2015-24

1. Assessment of Competence (Basic Training, Proficiency in Survival Craft and Rescue
   Boats other than Fast Rescue Boats, Advance Fire Fighting, Medical First Aid and Medical
   Care
2. Approved Seagoing Service or its approve alternatives
3. Proof of Successful Completion of Training in MLC-D
4. Valid Medical Certificate, which shall not be more than 2 years from the date of issuance,
   from DOH-Accredited Clinic/Hospital.
5. Proof of successful completion of OIC Updating Training Part B. (For BSMT/BSMarE graduates
   whose education and training started BEFORE June 2013)
