What are OIC-NW documentary requirements for Revalidation?

Reference: STCW Circular No. 2014-08, STCW Circular No. 2015-03, STCW Advisory No. 2015-21, STCW Advisory No. 2015-24

1. Assessment of Competence (Basic Training, Proficiency in Survival Craft and Rescue Boats other than fast rescue boats, Advance Fire Fighting and Medical First Aid)
2. Approved Seagoing Service
3. Valid Medical Certificate, which shall not be more than 2 years from the date of issuance, from DOH-Accredited Clinic/Hospital.
